Meta-analysis of risk factors for lymph node metastasis in T1 colorectal cancer

Background: Lymphatic vessel invasion (LVI) is precisely identified with anti-human podoplanin antibody (D2-40etc). However, in our knowledge, only small retrospective studies on LVI of D2-40etc as a risk factor for lymph node metastasis (LNM) in T1 colorectal cancer have been conducted and there is no higher evidence. This study aims to elevate the evidence on LVI that influences LNM in T1 colorectal cancer, meta-analyzing LVI of D2-40etc and other histopathological predictive factors. Patient and Method: Pubmed and Cochrane Library and manual reference checks were employed to search the studies from Jan.1998 to Des.2012 , utilizing combination headings, colorectal cancer, LNM, risk factors, with the limit of English language. Inclusion criteria were as follows: (1) Enrolled patients underwent bowel resection and were histologicaly diagnosed to have T1 colorectal cancer. (2) LVI must be identified by D2-40etc, separately from conventional H&E stain. (3) Univariate and multivariate analysis must be conducted in selected each study. The studies that investigated molecular markers were excluded. The independent predictive factors confirmed in selected each individual study were meta-analyzed in present study. Microsoft Excel 2000 for windows was used for the statistical analysis and Mantel-Haenszel odds ratios (ORmh) with 95% confidence interval (CI) were reported as the summary statistic. Result: 369 articles were searched from the databases and manual checks, and 5 literatures met the criteria of review and were chosen for this study. The meta-analysis revealed that the 5 predictive factors had statistical significance. LVI of D2-40etc; ORmh 5.19 (95% CI 3.31-8.15), Blood vessel invasion (BVI); 2.49 (1.55-4.00), Budding; 7.45 (4.27-13.02), Lower Tumor grade; 3.37 (1.54-7.34), Lymphatic vessel density (LVD); 2.09 (1.14-3.87). Conclusion: LVI of D2-40etc, BVI, Budding, Lower Tumor grade and LVD were significantly associated with LNM in T1 colorectal cancer.
