Dialysate lavage: an alternative to reduce peritoneal seeding of gastric cancer

Background: Peritoneal lavage after cancer surgery is to reduce microscopic residual tumors in peritoneum. This study evaluated the effect and mechanism of dialysate lavage in reducing peritoneal dissemination of gastric cancer. Methods: Gastric cancer cells (MKN45) were cultured with 1.5% peritoneal dialysate (PD) or normal saline (NS) for 30 minutes. The in vitro susceptibility to dialysate was evaluated by the MTT assay and flow cytometry. A murine model of peritoneal seeding of gastric cancer was established for testing the effects of PD or NS lavage on animal survival and tumor growth. Results: Significant decrease of cell proliferation (12.4 % versus 75.2%), increase of mitochondrial permeability transition (93.0% versus 18.0%) and cell death (25.6 % versus 16.9%) were recorded in MKN45 cells exposed to PD and NS, separately. In vivo study showed the average number of seeded tumors was 67.3, 92.3, 29.2, and the total weight of tumors was 0.98 g, 0.58 g, 0.31g separately in mice receiving sham injection, NS lavage and PD lavage at 24th day after inoculating MKN45 cells in their peritoneal cavity. The 45th day survival rates of PD lavage group was 22%, compared to 0% in sham injection and NS lavage, Conclusion: The use of dialysate lavage was effective in reducing peritoneal dissemination of gastric cancers. The cytotoxic effect of dialysate was in part related to the mitochondrial perturbation.
