Combined APC and Bougienage via endoscopy for treatment of anastomotic-stenosis

Background and Aim. Anastomotic-stenosis after esophageal cancer surgery are common and often refractory to treatment. We investigated the efficacy and safety of dilation of anastomotic-stenosis after esophageal cancer operation by using Argon Plasma Coagulation (APC) combined with Savary bougienage (SB). Methods. 90 patients with anastomotic-stenosis were treated with APC, SB alone or APC combined with SB. Patients were followed until 3 months after interventions. Data were compared using chi-squared test. Results. No sever complications occurred with three treatments. 1 week after treatment, there was no significant difference between the Combination group and the others groups on the effective rates (CR+PR) (Combination (100%) vs. APC (83.3%), P = 0.06, SB (100%) = Combination); 1 month and 3 months after treatment, the effective rates of Combination group significantly higher than the both APC and SB (1 month: Combination (90%) vs. APC (63.3%), P = 0.03, Combination vs. SB (16.7%), P = 0.00; 3 month: Combination (83.3%) vs. APC (50%), P = 0.01, Combination vs. SB (6.7%), P = 0.00). Conclusions. In esophageal squamous carcinoma patients with dysphagia caused by anastomotic-stenosis after esophagogastrostomy, the dilation treatment by APC combined with SB was significantly more efficient than APC or SB alone.
