初発髄膜腫に対する開頭腫瘍摘出術後早期の体重減少には身体活動の制限が関与する

Body weight loss in patients with cancer is associated with prolonged hospitalization and shorter survival after tumor resection. Patients with brain tumors are more likely to have limited nutrition and physical activity with reduced skeletal muscle mass and body weight. This study was performed to identify the factors associated with body weight loss during the postoperative hospital stay after surgical removal of primary meningiomas.
Twenty-five patients who underwent surgical resection of primary meningiomas from January 2017 through December 2019 were retrospectively investigated in terms of their basic information, pathological diagnosis, tumor grade and location, operation time, intraoperative blood loss, body weight during hospitalization, rate of change in body weight, postoperative hospital stay, nutrition-related blood data, nutritional intake, and performance status (PS). The rate of change in body weight was obtained by dividing the weight at discharge by the preoperative weight. This study was approved by the ethics committee of the Kagawa University Faculty of Medicine (approval No. 2019-199).
The patients' body weight was significantly lower at discharge than preoperatively (55.3 ± 8.3 vs. 57.0 ± 9.0 kg, respectively; p < 0.01). The mean rate of change in body weight was -2.7% ± 2.2%, and the mean postoperative hospital stay was 15.8 ± 4.0 days. The rate of change in body weight was significantly higher in patients with than without maintenance of their preoperative PS (-2.2% ± 1.8% vs. -4.0% ± 2.4%, respectively; p = 0.04).
This study revealed that lower physical activity at discharge than preoperatively accelerates weight loss after tumor resection for primary meningioma. In these patients, physical activity may have decreased because of continued restriction of activities of daily living after tumor resection, which may have led to a decrease in skeletal muscle mass.
