Background: Although there are several studies which compared colorectal cancer (CRC) patients who had anastomotic leakage (AL) with CRC patients who had not AL for postoperative survival, background factors were not aligned in most studies.
Objective: To explore influence of AL on postoperative survival in CRC patients.
Methods: We performed a comprehensive electronic search of the literature (The Cochrane Library, PubMed and the Web of Science) up to March 2020 to identify propensity score matching (PSM) studies that had compared postoperative survival between CRC patients with and without AL. A meta-analysis was performed using random-effects models to calculate the risk ratio (RR) and 95% confidence interval (CI), and heterogeneity was analyzed using I2 statistics.
Results: Four PSM studies involving a total of 1676 CRC patients undergoing surgery were included in this meta-analysis. Among 234 CRC patients with AL, 163 (69.7%) survived at 5-year after surgery, whereas among 1422 CRC patients without AL, 1156 (81.3%) survived at the same time. The results of meta-analysis revealed that there was a significant difference between the two groups (RR, 1.63; 95% CI, 1.09 - 2.45; P = 0.02; I2 = 66%) in 5-year overall survival (OS).
Conclusions: This meta-analysis using PSMs has clearly disclosed that AL significantly decreased 5-year OS in CRC patients with AR in comparison with CRC patients without AL.
