Objective: Peritoneal metastasis frequently occurs in patients with gastric cancer (GC). Intraperitoneal chemotherapy (IPC) using paclitaxel (PTX) combined with systemic chemotherapy has demonstrated a remarkable clinical efficacy for gastric cancer with peritoneal metastasis. However, there is no reliable biomarker associated with peritoneal metastasis. In this study, we examined exosome miRNA profiles derived from peritoneal fluids in patients with gastric cancer and explored biomarkers associated with IPC.
Methods: Peritoneal fluids were obtained laparoscopically from patients with advanced GC who underwent staging laparoscopy. Exosome fractions were isolated using ultracentrifuge method and the presence of exosome was confirmed with nanotracking analysis. Total RNA including small RNA was extracted from exosome fractions and expression of miRNAs were analyzed using RT-PCR. In 43 patients with PM who received chemotherapy comprised of systemic S-1/Oxaliplatin together with IPC using PTX, the relation between their relative expression level of miRNAs and clinical outcome was examined.
Results: The ratios of miR-21-5p/miR-29b-3p and miR-223-3p/miR-29b-3p were significantly upregulated in patients with PM compared with patients without PM. In 43 patients who received IPC, these ratios were significantly higher in 16 patients who showed apparent progression in PM within 1 year than other 27 patients who showed excellent tumor response. Overall survival of the patients with high miR-21/miR-29b or miR-223/miR-29b ratios was significantly worse than those with low ratios.
Conclusions: Exosomal miRNA in peritoneal fluids might be useful biomarkers to predict the response against IPC.
