リンチ症候群女性における悪性疾患発生状況の解析

We analyzed the clinical courses of women diagnosed with Lynch syndrome (LS) at the National Cancer Center Hospital, Tokyo, Japan to investigate the incidence of malignant disease. In total, 106 women diagnosed with LS between 1998 and 2019 were analyzed.
Seventy-seven women (72.6%) had malignant diseases: colon cancer (n=62), endometrial cancer (n=51), gastric cancer (n=16), ovarian cancer (n=12), other gastrointestinal cancer (n=11), urological cancers (n=10), breast cancer (n=7), cancers of biliary tract (n=5), pancreatic cancer (n=3), brain tumors (n=3), skin cancer (n=2), cervical cancer (n=1), and peritoneal cancer (n=1). The median age of patients at the onset of main disease is shown in the parenthesis: colon cancer (45.5 years), endometrial cancer (46 years), gastric cancer (56.5 years), ovarian cancer (45 years), other gastrointestinal cancer (60 years), and urological cancer (58.5 years). The initial disease was colon cancer (n=37), endometrial cancer (n=31), ovarian cancer (n=7), gastric cancer (n=4), biliary tract cancer (n=2), other gastrointestinal cancer (n=1), and breast cancer (n=1). (Some cases had double primary diseases.) Of the 62 women who had multiple malignancies, 17 had a history of at least five episodes. The total number of suffered organs were six organs (n=1), five organs (n=4), four organs (n=6), three organs (n=20), two organs (n=28), and one organ (n=18). Of the 37 women without malignant disease at the time of diagnosis with LS, eight suffered from malignant disease following the diagnosis. Of the 29 patients not confirmed to have developed the disease, 12 had not visited our hospital for more than 2 years, and the detailed course could not be determined.
Although this survey involved a small population, it revealed that LS has a high incidence rate. Therefore, it is important to continue the surveillance of LS.
