A 40-year-old woman was admitted to our department with fullness in the upper abdomen. Abdominal Ultrasonography, Computerized tomography and magnetic resonance showed a pancreatic tumor (12cm), multi-liver tumors, bone metastasis and swelling lymph nodes in para-aortic area. EUS-FNA from pancreatic tumor showed the neuroendocrine carcinoma and acknowledged findings of ki-67>20% , 3/10 HPF of mitotic counts pathologically. It is controlled by octreotide LAR 30mg/day once a month and the following chemotherapy: platinum-based regimen (TTP: 7.8 months) in 1^st line, Streptozotocin/5-fluorouracil (TTP: 7.9 months) in 2^nd line, 3^rd line Eberolimus (TTP: 2.8 months) in 3^rd line, Sunitinib in 4^th line. TAE(transcatheter arterial embolization) twice make several advanced multi-liver metastases shrinked. We report the case that the treatment of NET and NEC is effective for pancreatic neuroendocrine neoplasm.
