非小細胞肺癌における血漿エクソソーム内包microRNAのバイオマーカーとしての有用性

Objective: The aim of this study was to clarify the usefulness of plasma exosomal microRNA-451a (miR-451a) as a novel biomarker for early prediction of recurrence and prognosis in non-small cell lung cancers (NSCLC) patients after curative resection. Patients and Methods: Before surgery, plasma samples were collected and exosomal microRNA (miRNA) levels were evaluated. Using the miRNA microarray, we profiled exosomal microRNAs (miRNAs) related to the recurrence of NSCLC patients with pathological stage (stage) IA. We then validated the usefulness as biomarkers of selected miRNAs using the 285 NSCLC patients with stage I, II or III. Results: In the miRNA microarray analysis of plasma exosomes, miR-451a showed the highest up-regulation in NSCLC patients with recurrence as compared with miRNAs of NSCLC patients without recurrence and healthy controls. Plasma exosomal miR-451a levels of NSCLC patients were significantly higher than those of healthy individuals. A positive significant correlation was demonstrated between plasma exosomal miR-451a levels and NSCLC tissues miR-451a levels. Exosomal miR-451a showed a significant association with lymph node metastasis, vascular invasion and p-stage. The miR-451a levels in patients with stage III increased significantly than those with stage I. In stage I, II or III patients, overall survival (OS) and disease-free survival (DFS) rates of the high exosomal miR-451a patients were significantly worse than those of the low exosomal miR-451a patients. In Cox multivariate analysis, exosomal miR-451a was an independent prognostic factor for OS and DFS. Conclusion: The plasma exosomal miR-451a might serve as a reliable biomarker for prediction of recurrence and prognosis of NSCLC patients with stage I, II or III.
