The dynamic changes of the epithelial and mesenchymal composition of circulating tumor cells in breast cancer patients treated with Neo-adjuvant Chemotherapy

Background: CTCs(circulating tumor cells) were reported to be correlated to the prognosis and outcome of breast cancer patients. Recently, the consistent dynamic changes of epithelial and mesenchymal composition of breast cancer CTCs were reported. Doxorubicin was known to induce EMT and to increase the migration ability of MCF7 cells. However, it remain elusive whether chemotherapy can induce EMT in CTCs.
Methods: Peripheral blood mononuclear cells of 35 breast cancer patients treated with neo-adjuvant chemotherapy(NAC) were isolated in this prospective study. The CD326+ fraction and CD326-CD45- fraction of peripheral blood mononuclear cells were enriched using magnetic beads conjugated with CD326 antibody and CD45 antibody. Real-time quantitative PCR platform was used to detect epithelial-to-mesenchymal transition inducing tracscription factors (Twist1, Slug, Snail, FOXC2)and hMAN in CD326-CD45- fraction cells as surrogates of CTCs undergoning EMT (EMT-CTCs). On the other hand, the expression of CK19, SBEM, hMAN were assessed in CD326+fraction cells to indicate the existence of NON-EMT-CTCs. The dynamic CTC status of the epithelial and mesenchymal composition at two time point was observed.
Results: Non-EMT-CTCs were detected in 83.3% of patients(25/30) before NAC and only in 16.7% of patients(5/30) after NAC (p=0.000). While, EMT-CTCs were detected in 6.67% of patients(2/30) before NAC and 40% of patients(12/30) after NAC (p=0.013). No statistic significant correlation between the clinical characteristics and detectable Non-EMT-CTCs before and after NAE were observed. No statistic significant correlation between the clinical characteristics and detectable EMT-CTCs after NAE was observed.
Conclusion: In summary, NAC can eliminated Non-EMT-CTCs but not the EMT-CTCs. On the other hand, the NAC might induce the EMT of CTCs. Thus additional treatment to reverse the EMT of CTCs in patients treated with NAC is potentially needed.
