Advances in the multimodal treatment for rectal cancer

The treatment of the patient with non-metastatic rectal cancer has focused primarily on local tumor control. Advances in clinical staging, surgical techniques and adjuvant radiation therapy have reduced the risk of local recurrence significantly. The main threat facing rectal cancer patients today is the development of metachonous distant metastasis thought to develop from micro-metastasis clinically occult at the time of diagnosis. Therefore, the main challenge for those treating rectal cancer is to improve patient's survival by eradicating these micro-metastasis while achieving local tumor control and preserving quality of life. My presentation will review the evidence regarding the rationale for the novel neoadjuvant approaches that may improve outcomes by treating the micro-metastasis earlier, increase compliance with the treatment protocol, and enhance local tumor response thus opening the possibility of eliminating other forms of therapy - such as radiation or even surgery - from the multimodal treatment paradigm.
