Perceived breast cancer risk, objective risk and screening utilization in a urban city Tianjin, China

Objective - To evaluate women's breast cancer risk perceptions and determine the factors associated with accuracy of risk perceptions and screening utilization.
Methods - Data was collected in a cross-sectional survey, examined in 1526 women aged 20 to 69. Screening utilization included the ever-use of clinical breast examination (CBS), mammography and ultrasonography. Perceived risk was measured with the absolute risk item while objective risk was determined by health risk appraisal (HRA) model.
Results - Fifty-six percent women underestimated their breast cancer risk according to the HRA model. Women who born in urban areas (OR=0.657, 95%CI=0.475-0.909), having higher education level (OR=2.141, 95%CI=1.493-3.070), having a history of breast disease (OR=13.030, 95%CI=9.435-17.996), with family history of breast cancer (OR=2.310, 95%CI=1.177-4.535) and already postmenopause (OR=5.457, 95%CI=3.840-7.754) were more likely to underestimate their risk of breast cancer.
Eighty-one percent women had attended screening at least once by using clinical breast examination, ultrasound or mammography. Factors associated with screening utilization were urban areas (OR=0.483, 95%CI=0.345-0.677), higher education level (OR=0.379, 95%CI=0.252-0.570), personal history of breast disease (OR=5.408, 95%CI=3.617-8.087) and age of menarche (OR=0.516, 95%CI=0.355-0.751).
Conclusion - Breast cancer risk perception among Chinese women was unsatisfying. Women participating are overly optimistic, which may lead them to miss the opportunities to attend screening and neglect to take preventive measures. Health education on breast cancer risk was of great urgency among women, particularly in women living in cities and postmenopausal women. To promote the screening utilization, more attention should be focused on the vulnerable individuals, especially women born in rural areas, having a lower education level, without personal history of breast disease or whose age at menarche after twelve years.
