Molecular and genomic characteristics of circulating tumor cells (CTCs), and usefulness as biomarker for colorectal cancer patients

Background: Circulating tumor cells (CTCs) are promising biomarkers as a pioneer of liquid biopsy for cancer patients. However, molecular and genomic characteristics of CTCs at single-cell level are still unclear. We investigated molecular and genomic characteristics of CTCs, and clinical impact in patients with colorectal cancer (CRC). Patients and methods: A total of 201 CRC patients were studied. CTCs were collected from tumor drainage vein blood and peripheral blood of cancer patients. The phenotypes of CTCs were detected by Confocal quantitative image cytometer. Single cell was picked up by the Dielectrophoretic microwell array system, and applied for next generation sequencing after amplification of the DNA. Results: The number of CTCs with tumor markers (CEA, Cytokeratin) showed a significant increase with the progression of tumor stage. Furthermore, CTCs with cancer stem cell markers (CD133, CD44) and EMT markers (Vimentin, PLS3) showed a significant increase in the patients with advanced stage. Overall survival and disease-free survival of CTC-positive patients were significantly worse than those of CTC-negative patients. In the next generation sequencing analysis, heterogeneity of DNA mutations was found at the single cell level of CTCs. Conclusion: CTCs are promising prognostic biomarkers for CRC patients and effective tool for monitoring the tumor phenotypes.
